You must be under a doctor’s care and have a plan of care established and reviewed regularly by a doctor. Our agency will work closely with your doctor on a plan customized for you. For example, you have trouble leaving your home without help (like using a cane, wheelchair, walker or crutches; need special transportation or help from another person) because of an illness or injury. Or maybe leaving home isn’t recommended because of your health condition.
